When completing the Walgreens VAR (Vaccination Administration Record Informed Consent for Vaccination Form), please complete the following:

• Section A
• Section B # 1-10
• Section C  read and sign
• Section D
  • with medical card insurance information
• Upon completing the VAR-Informed Consent for Vaccination, you must print & sign your form to bring to the clinic.
