Employees Information for 2023 Flu Vaccination

 

RBHS New Brunswick/Piscataway & RWJMS employees:

• Free flu vaccine will be offered in Piscataway @ EOHSI with:
  • A Valid RU ID
  • Starting September 28th from 1:00-3:00 pm only
    • Every Thursday
    • Vaccine will not be given apart from the above assigned times.
    • No appointments are necessary for these clinic days.
    • For faster service and decrease in wait time, please fill out either the:
      • EOHSI Flu Consent / Declination Form  prior to arrival.
        •   OR
      • EOHSI Flu Consent form for >65 yrs old prior to arrival.
    • Short sleeves are appreciated

• There are also RWJMS  wellness fairs where flu vaccinations will be available:

• If you are a Covered Individual under the RBHS Immunization Policy for Covered Individuals (Policy 100.3.1), please ask for Proof of Immunization so you can upload it to the Vaccination website or scan the QR code below to upload:

 

RBHS Newark employees:

• NJMS Occupational Medicine Service (OMS) is offering flu vaccinations to RBHS Newark faculty and staff as follows:

• NJMS-OMS will be offering vaccination at the UH Cafeteria from 11 AM to 3 PM on the following Thursdays:
  • • 09/28/2023, 10/05/2023, 10/12/2023, 10/19/2023, 10/26/2023, 11/02/2023, 11/09/2023, 11/16/2023

 

• NJMS Occupational Medicine Service (located in the Stanley S. Bergen Jr. Building, 65 Bergen Street, Suite GA-167, Newark, NJ 07107) is also offering flu vaccinations to RBHS-Newark staff and faculty as follows:

  Flu Vaccination Clinic Days:

  • Walk-in Clinic:

    • Mon-Wed-Fri from 8:00-9:00am on a first-come first-serve basis.

    • The last person will be taken at 8:50am so please arrive early.

  • By Appointment:
    • Please call 973-972-2900 or email njms_occmed@njms.rutgers.edu (and someone will get back to you in 24 hours during normal business hours to schedule)
    • Employees should:
      • Bring their Rutgers ID
        • If there are questions on eligibility to get your vaccine through OMS, please call first.

      • Prior to your arrival

        • Complete the below Vaccine Checklist screening form and review the Vaccine Information Statement . If there are any questions related to flu vaccinations, please call or email. Thank you.
