Health Forms and Insurance\u00a0(<\/span>due July 6th, 2026<\/span>)<\/span><\/strong><\/h5>\n

After activating your NetID, go to\u00a0https:\/\/rutgers.medicatconnect.com<\/a>\u00a0to submit your Mandatory Health Forms. Select “Rutgers, The State of New Jersey” as your affiliated location. Detailed instructions for submitting Health Forms were sent via email and can also be found below or <\/span>here.<\/a><\/p>\n

Check your immunization status and schedule necessary appointments ASAP.<\/strong> Some of the required vaccines must be taken in multiple doses, so begin your immunizations immediately if you have not yet started. Concerning PPD testing, your private physician should administer and read the first PPD. You will be given the second part (administration and reading) of the PPD by the Student Health Service during Orientation Week. \u200b<\/p>\n

You will be required to comply with the University’s student immunization policy prior to beginning classes in July. \u00a0 A copy of the policy is available to you on online student health portal, but I have taken the liberty of listing the requirements for compliance with this immunization program.\u00a0 \u00a0These requirements are as follows:<\/p>\n

    \n
  1. Tuberculosis (TB):\u00a0 \u00a0<\/strong>A two-step PPD (Tuberculin test)\u00a0(1-3 weeks apart<\/strong>) or FDA approved IGRA test (eg TB Gold Quantiferon or T-spot) is required within 6 months of matriculation.\u00a0 If you test positive, further PPD counseling and\/or treatment will be provided to you. If you have already done further evaluation with your private physician, please provide that documentation as well.<\/li>\n
  2. Measles-mumps-rubella:\u00a0\u00a0 \u00a0<\/strong>You will\u00a0have\u00a0to present\u00a0documented proof of your immunity to measles, mumps and rubella (MMR). This can be accomplished by:\n
      \n
    1. serologic (laboratory) evidence of immunity to each disease;<\/li>\n
    2. documentation by a physician or public health department of receipt of two doses of live-virus measles vaccine, one dose each of mumps and rubella vaccine. \u00a0The first dose of each must have been given on or after your first birthday. The second dose of measles vaccine must be given no less than one month after the first dose.<\/li>\n<\/ol>\n<\/li>\n
    3. Tetanus-diphtheria-pertussis:\u00a0 \u00a0<\/strong>You must have completed a primary series.\u00a0 \u00a0All students who have not had a Tdap booster in the past 10 years are required to have Adacel or Boostrix(Tetanus Toxoid, Reduced Diphtheria Toxoid and Acellular Pertussis).<\/li>\n
    4. \u00a0Polio: \u00a0<\/strong>You must have had an immunization series for all types of polio (3 dose primary series).A Booster Polio is acceptable. Medical documentation must be provided.<\/li>\n
    5. Varicella:\u00a0 \u00a0<\/strong>You must show evidence of immunity for chicken pox (varicella zoster) by serologic testing or documentation of receiving a series of two doses of varicella vaccine 4 to 8 weeks apart. In the absence of evidence of immunity, you must receive a series of two doses of varicella vaccine 4 to 8 weeks apart. \u00a0If you have begun the series, the Student Health Service will be prepared to complete the series. \u00a0The costs of these immunizations are not included in the student health fee and will be added to your term bill.<\/li>\n
    6. Hepatitis B: <\/strong>You must show that you are immune from Hepatitis B and not already infectious with Hepatitis B. Please provide documentation that you have had the\u00a0series of three vaccinations\u00a0<\/strong>against Hepatitis B. The first two vaccinations should be given one month apart. The third vaccination must be given at least 4 months after the first dose, and at least 2 months after the second dose. If you have not completed the series of vaccinations, the Student Health Service will be prepared to administer the series. You will be charged for the cost of these immunizations. You are also required to have serologic proof of immunity by obtaining a\u00a0quantitative Hepatitis B Surface Antibody\u00a0<\/strong>test. This should be performed at least 1 month after the last Hepatitis B immunization.<\/li>\n<\/ol>\n

      If your Antibody test does not show immunity to Hepatitis B then you must get Hepatitis B Surface Antigen test done. Options if you are not immune to Hepatitis B include having a Hepatitis B booster done followed by repeat titers one month after or repeating the Hepatitis B series followed by repeat titers one month after.\u00a0 Additional testing may be required as deemed appropriate by the Student Health Service. If these tests indicated potential for transmission, an evaluation shall be made of the need for monitoring clinical performance and the scope of assigned or permitted clinical activities consistent with patient protection.<\/p>\n

        \n
      1. COVID-19 Vaccination is no longer required.\u00a0 Please be aware there may be some clinical sites that still require this vaccination.\u00a0 The registrar will inform of these requirements when scheduling clerkships, if they exist.<\/li>\n<\/ol>\n

        These requirements have been implemented to protect both you and patients that you will have contact with during your studies at Rutgers Robert Wood Johnson Medical School. \u00a0If for religious, health, or other reasons, you have not been completely immunized, please provide official verification for requesting an exemption from this requirement. Medical exemptions require a physician’s verification. \u00a0Should you have any questions about your status regarding the required immunizations, please call Student Health Service, (732) 235-5160 or email Jennine Boruta, LPN, at\u00a0fammedstudenthealth@rwjms.rutgers.edu . \u00a0<\/a>A health care provider will return your call to review your status with you. \u00a0Please make sure to include a cell phone number with which you can be reached.<\/p>\n

        Student Health Service staff will be present during Orientation. \u00a0If we can assist you in any way, please do not hesitate to call. \u00a0I look forward to greeting you in the near future.<\/p>\n

        You can access Student Health Portal online at\u00a0https:\/\/rutgers.medicatconnect.com\/<\/a>.\u00a0Immunization Requirements must be met prior to matriculation. \u00a0If you do not meet these requirements, you will not be permitted to enroll.<\/strong><\/p>\n

        The student Health Portal is not available until you have activated your netID.<\/p>\n
