Faculty/Staff Referral Form (Tutoring & Academic Coaching)
To ensure that our students have access to the necessary support for their academic progression and success, we encourage you to utilize our faculty/staff referral system to refer students to the Office of Academic Success. If a student has yet to meet the standards for academic progress in their program, a faculty/staff referral can provide them with valuable resources and assistance.
Common Referral Areas:
- Test-taking strategies
- Study strategies
- Managing test anxiety
- Tutoring
Faculty and staff are critical in identifying students who may benefit from additional academic support. By completing a faculty referral form for a student, you are helping them to bridge the gap between their current academic performance and their potential.
Process for Referrals:
- Complete the Faculty Referral Form.
- Provide details on the specific areas where the student may benefit from learning assistance.
- Submit the referral form to the Office of Academic Success.
What Happens Next:
– The student will receive an email detailing the available learning assistance services and resources.
– The email will also include the reason why a faculty or staff member made a referral for the student.
Faculty Nominated Peer Tutor Program:
Qualifications
To qualify as a Faculty Nominated Peer Tutor, students must meet the following requirements:
- Must be a rising Sophomore or Level 2 student
- Must have a 3.5 or higher cumulative grade point average
- B or higher in all nursing courses
- No academic or professional integrity violations
- Receive nomination support from a nursing faculty member.
Recruitment
The Office of Academic Success invites faculty to nominate prospective peer tutors each semester for their courses. The faculty nomination form will open during the time frames listed below and remain open for the remainder of the semester.
Fall Semester: Late September
Spring Semester: Early February
Summer Semester: Early June

After submitting a faculty referral, the student(s) will receive an email with a recommendation for five touch points with our office. This could include tutoring, academic coaching, success workshops, peer academic coaching and self-guided resources.
If a faculty member would like to follow-up on a referral please reach out to OAS. OAS will provide a list of services the student(s) has used.
