Please complete this Mandatory Student Intake Form by 11:59 pm on June 15. This will be used during Orientation Week, as well as to order your white coat.

Please also take the time to fill out the M1 Questions and Concerns form. While it is optional, it will help us better plan Orientation Week and parts of the curriculum throughout the academic year. All responses to this form will be anonymous.
